18-Apr-1990 - Crashed into Caribbean shortly after take-off from Isla Contadora, Panama following multiple bird strikes (Parrots). The failure of the No. 2 engine caused the aircraft to enter a descending turn which continued until impact with the sea. There were 20 fatalities and 2 survivors.
