17-Jun-1979 - Crashed into heavily wooded area 1.5 miles northeast of Barnstable Airport, Hyannis MA while on approach. Subsequent investigation determined the cause to be a combination of gross deviation from acceptable approach parameters and lack of crew coordination. There was just a single fatality, the aircraft Captain – the Vice President of Air New England.
