12-Jul-1995 - Caught fire and crashed immediately after take-off from the Gurney Airport at Alatau, Papua New Guinea. Subsequent investigation linked the source of the fire to be electrical, combined with kerosene which was being carried in passenger luggage. There were 15 fatalities.
