28-Oct-1989 - Collided with a mountain ridge on northeastern shore of the island of Molokai, while en route from Kahului, Maui to Kaunakakai, Molokai. The subsequent investigation determined the primary cause as continued visual flight into IMC conditions. There were 20 fatalities.
