05-Jul-2004 - On take-off from a 1,500ft gravel strip at Mile 222 of the Canol Road (west of Norman Wells) the aircraft departed the left side of the runway at approximately 60 knots, coming to rest in an adjacent creek. There was substantial damage to the landing gear and the right wing separated from the fuselage.

The wreckage was subsequently recovered and transported to the Rocky Mountain Aircraft facility at Springbank.
